TOURIST REQUIREMENTS

  • Original signed passport- must be at least 6 mo. valid with 2-3 clear visa pages

  • Two passport photos (2"x2")- signed by the applicant on the back- must be taken within last 2-3 months

  • Two Application Forms (click here to download)

  • One IVS Order Form (click here to download)

  • Copy of round trip itinerary

  • Yellow Fever Vaccination Card (copies accepted)

  • Copy of Green Card for non-US citizens


BUSINESS REQUIREMENTS
