STEP 1. Gather the following documents according to the purpose of your trip:


TOURIST & BUSINESS REQUIREMENTS

  • Copy of US passport information page and signature page- must be at least 6 mo.

  • One passport photo (2"x2")- must be taken within last 2-3 months

  • One Application Form (click here to download)

  • One IVS Order Form (click here to download)

  • Copy of round trip itinerary

  • Yellow Fever Vaccination card (copies accepted)

  • Copy of Green Card for non-US citizens

Due to the high volume of applications being submitted, the processing times may be delayed by the Passport Agency.
