Use this procedure if:

--Your US Passport card was stolen, lost or misplaced

--You are applying for passport card only

--If you are applying for replacement of Passport Book & Passport Card at the same time, please use different procedure: for adults, for minors or for teens 16-18 years of age

Please note Int'l Visa Service is NOT a government agency. We are a private company that is authorized to process US Passport books and cards by the US Passport Agency. A service fee will be associated with each order in addition to the government and shipping fees. 


STEP 1. Gather the following documents:

• Copy of birth certificate or certificate of naturalization
  

• One passport size photo 2"X 2"  (must be taken within last 2-3 months), cannot wear glasses
  

• One Application DS-11 (click here to download)
  

• One Application DS-64 (click here to download)
  

• Copy of driver's license (front & back). For minors: copy of each parent's driver's license (front & back)
  

• IVS Order Form (click here to download)
  

STEP 2. E-mail copies of the documents to cs@ivsdc.com
Our passport specialists will review copies of your paperwork and will prepare final application packet for you.

STEP 3: Receive final application packet from us by e-mail with instructions on where to go, how much to pay and how to track your application.

*Please note that Int'l Visa Service will not be able to get involved after your original documents are mailed to the National Passport Processing Center or contact the Passport Processing Center on your behalf. If additional documents or information is requested by the government and you need assistance in understanding the communications, putting together additional paperwork and submitting, additional fees will apply.
