*Please note that Int'l Visa Service will not be able to get involved after your original documents are mailed to the National Passport Processing Center or contact the Passport Processing Center on your behalf. If additional documents or information is requested by the government and you need assistance in understanding the communications, putting together additional paperwork and submitting, additional fees will apply.

DAMAGED U.S. PASSPORT FOR TEENS AGES 16-18

Please note, the damaged passport will not be returned to you.
Use this procedure if:

  • Your child is between 16-18 years of age
  • Your child's passport's binding is coming apart
  • The information page of your child's passport is damaged (photo is not clearly visible, barcode is damaged)
  • Your child's passport was washed with laundry, chewed by a pet, used as a coloring book, etc.​



STEP 1. Gather the following documents:


  • Copy of birth certificate with both parents' names on it or certificate of naturalization

  • Copy of damaged US Passport (it will not be returned to you)

  • One passport photo (must be taken within last 2-3 months, cannot wear glasses)

  • One Application DS-11 (click here to download)

  • Statement from the applicant as to how the passport was damaged

  • Copy of driver's license or ID of the applicant (front & back)

  • Copy of driver's license of the parent who will go to the post offce with the teen (only for 16 & 17 yr old applicants)
  • IVS Order from (click here to download)



STEP 2. E-mail copies of the documents to cs@ivsdc.com

Our passport specialists will review copies of your paperwork and will prepare final application packet for you.



STEP 3. Receive final application packet from us by e-mail with instructions on where to go, how much to pay and how to track your application.
