  • Copy of signed passport- must be valid for at least 6mo with 2-3 clear visa pages

  • One passport photo in jpeg format- must be taken within last 2-3 months

  • Copy of Yellow Fever Vaccination Card

  • Copy of Green Card for non-US citizens
