Please note, all 2nd passports are valid for 4 years from the date of issuance. Passport card cannot be added to this order.

Use this procedure if:
--You are 16 or older
--You already have a regular 10yr passport

--If you are applying for the 2nd passport for the first time (if you need to renew 2nd passport, click here)
--You have extensive travel coming up
--You have Israeli stamp in your passport and are traveling to Middle East where Israeli stamp is not allowed

STEP 1. GATHER THE FOLLOWING DOCUMENTS:

• Original valid 10yr passport (must not be expired)
  

• Two passport photos 2"x 2"- (must be taken within last 2-3 months), no glasses
  

• One Application DS-82 (fill out online)
  

• Letter of Authorization (click here to download)
  

• 2nd Passport Statement (click here to download)
  

• 2nd passport request letter (click here to download)
  

• IVS Order Form (click here to download)
